What makes tirzepatide different
Every other GLP-1 medication acts on one appetite hormone. Tirzepatide acts on two — GLP-1 and GIP — which is why it produces the largest average weight loss of any medication currently licensed for weight management. In SURMOUNT-1, participants on 15 mg lost an average of 20.9% of body weight at 72 weeks.
The dual mechanism also changes the side-effect profile slightly. Nausea and fullness are broadly similar to semaglutide but the appetite suppression tends to be more pronounced. Neither is a reason to fear the drug; both are reasons to have a real GP titrating your dose.
Tirzepatide vs semaglutide, head-to-head
The SURPASS-2 trial compared tirzepatide directly against semaglutide 1 mg in type 2 diabetes and showed greater HbA1c reduction and greater weight loss for tirzepatide at every dose. For weight-management indications, tirzepatide 15 mg produces roughly 20–22% average loss at 72 weeks versus semaglutide 2.4 mg at 15–17%.
Whether that gap matters for you depends on your starting point, your tolerance for side effects and cost. A well-managed course of Wegovy will out-perform a badly managed course of Mounjaro every time — the medication is only half the story.
