Provided by: Carers working under the supervision of DHA-licensed Registered Nurses at Aafiyah Care Clinic
Doctor involvement: A GP reviews the care plan and sees the patient when a health concern is raised.
How do we choose the right carer for your mother or father?
A nurse visits first. They meet your family member, look at the home, ask about routines and preferences, and write a care plan you agree to before anyone starts.
We aim to keep the same carer for each visit so your family member gets to know them.
What is the difference between a carer and a nurse?
A carer helps with daily living. A nurse carries out clinical tasks such as injections, dressings and catheter care. With Aafiyah both work from one care plan, so you do not need two providers.
What happens if their health changes?
Carers report changes in eating, mood, mobility, breathing or confusion to the supervising nurse the same day. The nurse can visit, and our GP can review at home or in clinic.
