Real hypogonadism vs marketing
True male hypogonadism requires two morning (7–10am) fasting total testosterone readings below range, plus symptoms. SHBG, LH, FSH and prolactin help distinguish primary from secondary hypogonadism and rule out reversible causes.
Many clinics start TRT on a single reading or on symptoms alone. That is not evidence-based medicine — it is prescribing.
The Aafiyah pathway
Free 15-minute GP call. Baseline bloods: total and free testosterone (morning, fasting, x2), SHBG, LH, FSH, prolactin, oestradiol, HbA1c, lipids, PSA, haematocrit, ferritin.
If genuine hypogonadism is confirmed, TRT is discussed with informed consent covering fertility, haematocrit, cardiovascular, PSA and mood. Monitoring at 6 weeks, 3 months, then every 6 months. If not confirmed, we address the actual driver — sleep, weight, alcohol, stress — because these commonly cause low reads.
TRT and fertility
TRT suppresses natural sperm production. If fertility is a current or future goal, we discuss alternatives — clomiphene, HCG, lifestyle — before starting exogenous testosterone. This conversation is part of every TRT consultation.

What's a 'low' testosterone number?+
Typically total testosterone below 8–12 nmol/L with symptoms, confirmed on two morning readings. Free testosterone matters when SHBG is unusual.
Can weight loss raise testosterone naturally?+
Often, yes — significant weight loss raises testosterone in men with obesity-related low T. We usually address weight and sleep first when possible.
Is TRT safe long-term?+
With proper monitoring (haematocrit, PSA, lipids, blood pressure) and appropriate dosing, TRT has a well-characterised safety profile. Cardiovascular risk is followed carefully.
Do you offer HCG or clomiphene?+
Where fertility preservation is relevant, yes — alongside TRT or as alternatives.
Can I combine TRT with Mounjaro or Wegovy?+
Yes, commonly. Both work on different systems and monitoring covers both.
