Why thyroid comes first
Untreated or undertreated hypothyroidism lowers resting metabolism, increases fluid retention, worsens fatigue and blunts response to any weight-loss intervention. If your TSH is above range or your free T4 is at the bottom of range, that is your first problem — not the food.
Autoimmune (Hashimoto's) thyroiditis is the commonest cause. TPO antibody testing confirms it. Dosing needs to be individualised — 'in range' TSH is not the same as optimal for you.
The Aafiyah pathway
Free 15-minute GP call to review symptoms, current medication and history. Baseline bloods: TSH, free T4, free T3, TPO antibodies, plus a full metabolic panel (HbA1c, insulin, lipids, vitamin D, B12, ferritin, iron studies).
If thyroid is undertreated, your GP adjusts levothyroxine dose and repeats bloods in 6–8 weeks. Weight-loss programme starts once thyroid is optimised — with realistic expectations, appropriate nutrition and GLP-1 medication only when clinically appropriate.
Reverse T3, T3-only prescribing and honest medicine
Aafiyah practises evidence-based medicine. That means we don't chase reverse T3, we don't add T3 or NDT unless there's clear evidence it's needed, and we don't blame every symptom on thyroid when the picture doesn't fit. If T3 is genuinely indicated, we prescribe carefully — with monitoring.

My TSH is 'normal' but I feel awful — what next?+
Range is not the same as optimal. Free T4, free T3, symptoms and dose response all matter. Your GP reviews the whole picture, not just one number.
Can I take GLP-1 medication with hypothyroidism?+
Yes, once your thyroid is properly treated. GLP-1 has no direct thyroid contraindication for people without a history of medullary thyroid cancer or MEN2.
Should I take T3 or NDT?+
Only where clearly indicated and monitored. We prescribe evidence-first, not fashion-first.
How often are bloods repeated?+
Every 6–8 weeks after a dose change; every 3–6 months once stable; annually thereafter.
Do you treat Hashimoto's?+
Yes — we diagnose, treat and monitor. Immunomodulation (nutrition, vitamin D, selenium in selected cases) is part of the plan.
