What's actually changed
Muscle mass falls 3–8% per decade after 30 unless actively defended. Insulin sensitivity drops. Sleep becomes lighter. In women, perimenopause shifts fat storage to the abdomen. In men, testosterone drops around 1% per year. Cardiovascular risk emerges. Recovery slows.

Free 15-minute GP call. Full baseline: HbA1c, fasting insulin, lipids, ALT, GGT, kidney function, vitamin D, B12, ferritin, thyroid, hormones (testosterone in men; oestradiol/FSH/LH in women), body composition, blood pressure, cardiovascular risk score.
Plan: nutrition emphasising protein and insulin sensitivity, resistance training to defend muscle, sleep support, HRT or TRT where indicated, statin/BP treatment where risk dictates, GLP-1 medication where BMI and metabolic markers make it right.
GLP-1 in your 40s — used carefully
GLP-1 medication works well in your 40s. The key is titrating slowly, protecting muscle with adequate protein and resistance training, and tracking body composition rather than just scale weight. Losing weight is easy; losing fat while keeping muscle is what matters at this age.

Do I need HRT?+
If you're perimenopausal or menopausal and symptomatic, HRT is often the right foundation. Your GP discusses risk and benefit in detail.
Will GLP-1 make me lose muscle?+
It can if used carelessly. Slow titration, adequate protein, resistance training — done right, most weight lost is fat.
How often do I see the GP?+
Monthly reviews. Bloods at baseline and every 3 months.
Do you check cardiovascular risk?+
Yes — QRISK/Framingham calculation, blood pressure, lipids, HbA1c and family history all inform treatment choices.
