The common ones (usually manageable)
Nausea and fullness. Most patients feel this in the first 1–4 weeks and it usually settles as the body adapts to each dose. Smaller meals, slower eating and gentle food choices help. If it's persistent, your GP will pause the titration or hold your dose rather than push through.
Mild GI upset — reflux, constipation or loose stools. Water, fibre and simple dietary tweaks fix most of it. Occasionally a short-course PPI or laxative is used alongside.
Fatigue in the first weeks — often a signal of under-eating rather than the drug itself. Reduced appetite doesn't mean reduced protein or micronutrient needs; if intake drops too low, energy follows.
The ones that need attention
Gallstones. Rapid weight loss of any kind raises gallstone risk. GLP-1s are associated with this too. Persistent right-upper-abdominal pain, especially after fatty meals, needs a same-day GP call.
Pancreatitis. Rare but real. Severe upper-abdominal pain radiating to the back is a red flag — stop the medication and seek urgent care.
Muscle mass loss. Up to 25–40% of total weight lost on GLP-1s can be lean mass without protein intake and resistance training. This isn't a quirk; it's a long-term metabolic problem. Prevention: 1.4–1.8 g protein/kg body weight per day, resistance training 2–3× per week, tracked at your reviews.
