How cortisol drives weight gain
Cortisol is the survival hormone — it raises glucose for immediate energy and drives fat storage centrally for reserves. Chronic elevation (job stress, poor sleep, over-training, alcohol, undernutrition) creates the classic picture: abdominal weight gain, sugar cravings, poor sleep, fatigue despite rest.
We assess the pattern rather than chasing a single number. 'Adrenal fatigue' is not a real diagnosis, but chronic cortisol elevation absolutely is — and it is treatable.
The Aafiyah pathway
Free 15-minute GP call to map your sleep, stress load, work pattern, alcohol, training and symptoms. Baseline bloods: morning cortisol, HbA1c, insulin, lipids, thyroid, vitamin D, B12, ferritin, DHEA-S. Salivary cortisol day curve arranged in selected cases.
Treatment plan: sleep architecture work, stress-load reduction, nutrition and training tuned to lower cortisol load, and — only where clinically appropriate — GLP-1 medication. If Cushing's is suspected, we investigate properly and refer to endocrinology.
When to worry about Cushing's
Rarely, weight gain with a specific pattern (moon face, purple striae, easy bruising, muscle wasting, uncontrolled hypertension or diabetes) suggests Cushing's syndrome. Your GP screens with overnight dexamethasone suppression or 24-hour urinary free cortisol and refers to endocrinology if positive.

Is 'adrenal fatigue' real?+
No — it is not recognised in medicine. Chronic stress, poor sleep and hypocortisolism are real and treatable, but 'adrenal fatigue' as a diagnosis is not.
Do I need a cortisol test?+
Sometimes — morning cortisol at baseline, salivary day curve in selected cases. Symptoms plus metabolic bloods often tell us more than a single cortisol value.
Will GLP-1 medication help?+
It can help with the appetite and abdominal weight side, but it doesn't fix the underlying stress. We treat both together.
What if I can't fix the stress?+
We work on what's changeable — sleep, movement, nutrition, caffeine, alcohol, boundaries — and adjust expectations. Progress is possible even in high-stress lives.
Do you offer sleep support?+
Yes — sleep assessment is part of the programme, with referral for sleep study where sleep apnoea is suspected.
