Who do we really serve?

It's a question not enough people in healthcare ask themselves. Is it the government? The insurance company? The shareholders? The data? The profit margin?

At Aafiyah Care Clinic, we asked ourselves that question early on. Our answer was clear: we work for the patient. Always. Not for the paperwork. Not for the premium. Not for the protocol.

When the system gets in the way of care

Across the healthcare industry — not just in Dubai — clinicians are often forced to choose between doing what's right for the patient, doing what the system reimburses fastest, and doing what's convenient for throughput.

You end up with short appointments, reactive medicine, tick-box assessments and patients passed from one provider to another. And somewhere along the way, we lose the most important thing: trust.

Why this matters in Dubai's healthcare landscape

Let's be honest — Dubai has some of the best medical infrastructure in the region. But infrastructure doesn't equal care. And access doesn't guarantee understanding.

Here's what many patients tell us: 'I'm sent to too many different doctors.' 'I don't know who actually knows me.' 'I feel like I'm being processed, not cared for.' 'Every visit feels rushed.' That's not a reflection of bad doctors. It's a reflection of a system that prioritises structure over soul. We're changing that.

We've seen it in the UK. We're proving it in Dubai. The system becomes more efficient when it stops being the centre of attention.

Final thought: it's time to rehumanise healthcare

We're not anti-system. We're pro-patient. And if the system learns to serve the people, not itself, then it becomes sustainable, respected and genuinely excellent.

What 'working for the patient' rules out

Commission-based clinician pay. Quotas on diagnostics. Booking systems that penalise long consultations. Pharmacy upselling. Default referrals to in-house specialists you didn't ask for. None of these are uncommon in Dubai; all of them push the clinician's incentive away from your interests.

What it requires structurally

Salaried clinicians. Time-based consultations long enough to do the job. A referral network chosen on quality, not commercial agreement. A medical record the patient owns and can take with them. And leadership prepared to lose revenue on the visits where the right answer is 'you don't need that test'.

It sounds obvious. In a high-throughput private market it is rare.

Concrete examples from the clinic

Telling a worried parent their child's viral illness doesn't need antibiotics — and explaining why in plain English. Removing a routine annual scan that adds nothing for a low-risk patient. Picking a generic medication over a branded one where evidence is equivalent. Recommending against an in-house procedure if the patient would be better served elsewhere. Each is a small decision; cumulatively they define the model.
