A system at a tipping point

Healthcare in the UAE is evolving rapidly. Vision 2030, digital transformation and mandatory insurance schemes are reshaping how care is accessed, delivered and funded.

Yet at the centre of it all lies a fundamental question: are we delivering care in the most effective and sustainable way? As a healthcare leader with roots in the UK and eyes firmly set on Dubai's future, I believe the UAE is at a crossroads — and the path forward must start in primary care.

A system built around reaction, not prevention

Patients often bypass general practice and walk straight into specialist clinics. Referrals are inconsistent or absent. Preventive care is underused. Chronic conditions like diabetes and hypertension continue to rise, year on year.

Costs are escalating — not because of bad care, but because of inefficient pathways. In 2024 alone, UAE health insurance premiums spiked by up to 35%. Over 55% of all deaths in the UAE were attributed to non-communicable diseases. A single hospital admission for an avoidable chronic condition can exceed AED 16,000 — and yet many of these could be prevented through GP-led management.

This is not a blame game. It's a system design issue. And now is the time to solve it.

What the data tells us

The average annual cost per diabetes patient in the UAE ranges from AED 5,900 to AED 20,800. Around 50% of diagnosed diabetics still have poor glycaemic control (HbA1c >7.5%). A single inpatient admission averages over AED 16,000. Roughly 50–60% of total healthcare spend goes to chronic disease management. And before any of that, primary care is too often skipped entirely — there is no formal gatekeeping in the system.

The pattern is clear: we are treating late, referring early and paying heavily.

Primary care as the strategic front line

Primary care, when done right, is not a step down — it is a strategic control point. It is the one place in the system where risk factors can be identified early, patients can be tracked consistently, costs can be controlled without compromising outcomes, and specialists can be accessed appropriately rather than excessively.
