Vitamin D Deficiency in Dubai: The Sunny City's Hidden Problem

Why a city with year-round sun has one of the highest rates of vitamin D deficiency in the world.

Why deficiency is so common here

Indoor lifestyles, sunscreen, modest clothing, glass-fronted offices and an aversion to midday sun all reduce skin synthesis of vitamin D — even in the sunniest months.

Testing and treatment

A 25(OH)D blood test confirms levels. Target is 75–125 nmol/L. Loading-dose oral cholecalciferol corrects most deficiencies in 8–12 weeks; IM injections are reserved for malabsorption.

How low is too low?

Serum 25(OH)D below 50 nmol/L is deficient; 50–75 is insufficient; 75–125 is the optimal range we aim for in symptomatic patients. Below 25 nmol/L is severe deficiency and is linked to bone pain, muscle weakness, low mood and impaired immunity — and is genuinely common in Dubai despite the climate.

Why supplementation strategy matters

A daily 800 IU multivitamin will not correct a true deficiency in any realistic timeframe. We typically use a loading dose of 50,000 IU weekly for 6–8 weeks, then a maintenance dose of 1,000–4,000 IU daily depending on body weight, sun exposure and absorption. We retest at 3 months. Intramuscular injections are reserved for malabsorption (post-bariatric, coeliac, IBD) where oral routes fail.

What deficiency actually feels like

Persistent fatigue, vague muscle and joint aches, low mood, hair shedding, frequent minor infections, and reduced exercise tolerance. Many patients describe a flat, washed-out feeling that they've put up with for years. Correcting the deficiency rarely transforms someone overnight — but at 6–12 weeks most notice the difference clearly.

Who should test, and how often

Everyone living in Dubai should test at least once. Anyone with persistent fatigue, low mood, recurrent infections or chronic pain should test sooner. After correction, annual retesting is sufficient unless symptoms recur. Pregnant women, those on glucocorticoids, and patients with bowel disease need more frequent monitoring.

Food sources are not enough in Dubai

Fatty fish, egg yolks and fortified foods contribute a modest baseline of vitamin D but cannot correct an established deficiency. Even a daily oily-fish habit delivers only 200–600 IU — well below the 2,000–4,000 IU maintenance most deficient adults need. Supplementation, calibrated to retesting, is the practical answer for the vast majority of residents.

Vitamin D and immunity, mood and bone

Recent meta-analyses link replete vitamin D status to reduced respiratory infection rates, modestly improved mood scores and reduced fall and fracture risk in older adults. None of these are magic — but combined with the wide prevalence of deficiency in Dubai, they make a strong case for testing and correcting where indicated rather than guessing.

Children, pregnancy and the elderly

All three groups have higher requirements and lower tolerance for deficiency. Children need vitamin D for bone development; deficiency in this group still causes rickets, including in sunny climates. Pregnant women need replete vitamin D for fetal bone and neurological development. Older adults need it for fall and fracture prevention. We screen these groups proactively and supplement at age- and weight-appropriate doses.

Vitamin D toxicity is rare but real

Toxicity essentially only occurs with sustained supplementation above 10,000 IU daily without monitoring, and presents as hypercalcaemia — confusion, nausea, frequent urination, kidney stones. This is why we retest at 3 months after a loading dose and don't recommend indefinite high-dose supplementation without follow-up bloods. Most patients land on a maintenance dose between 1,000 and 4,000 IU daily once optimised, with annual checks thereafter to confirm levels remain in the target range without further adjustment.
