Thyroid, Iron or Hormones? Making Sense of "Just Tired All The Time"
Three quiet culprits, one underlying problem. Here's how a proper Dubai women's panel separates thyroid from iron from hormones — and what to do about each.
Three suspects, often guilty together
For women, persistent unexplained fatigue almost always traces back to one (or, more often, a combination) of three things: thyroid dysfunction, iron deficiency, and hormonal change. They overlap symptomatically. They feed into each other. And the only way to know which is doing the most damage is to test them all together.
Iron and ferritin — the most under-tested marker in women's health
Haemoglobin (the red blood cell count on a basic blood test) is the last thing to drop in iron deficiency — by which point you've already had months or years of low ferritin (your iron stores) causing fatigue, hair shedding, brain fog and exercise intolerance. The fix: test ferritin, not just haemoglobin. Healthy ferritin for an energetic woman sits above 50 µg/L; many women feel best above 80. Lab 'normal' can start as low as 15.
Thyroid — beyond a single TSH
Subclinical hypothyroidism is very common in women, particularly post-pregnancy and through perimenopause. A TSH alone isn't always enough; we also look at free T4, free T3 and thyroid antibodies where indicated. Symptoms of low thyroid — fatigue, weight gain, cold intolerance, low mood, hair changes — overlap heavily with iron deficiency and perimenopause, which is why testing them together is essential.
Hormones — not just an oestrogen story
FSH, LH, oestradiol, progesterone, SHBG and prolactin together tell the story. PCOS, perimenopause, hyperprolactinaemia and luteal phase dysfunction all show up differently in this panel — and all of them can cause fatigue, mood and weight changes. Interpreting them properly means looking at timing in the cycle, age, and the rest of the bloods alongside.

Why do women get low iron more often than men?
Menstrual blood loss, pregnancy and breastfeeding all increase iron requirements. Combined with diets often lower in red meat, iron deficiency is extremely common — and frequently the single biggest driver of female fatigue.
