Signs of Low Testosterone in Dubai Men: Symptoms, Testing and Real Treatment
Low testosterone is over-diagnosed in lifestyle clinics and under-diagnosed in regular GP practice. Here is the evidence-based middle ground.
The Dubai testosterone paradox
Walk into a longevity clinic and you'll be told most men over 35 have low testosterone and would benefit from replacement. Walk into a general practice and you'll be told your symptoms are stress, weight and sleep. The truth is in between, and getting the diagnosis right matters because testosterone replacement therapy (TRT) is lifelong and carries real risks.
This is the framework we use to assess men presenting with possible low testosterone.
Symptoms that should prompt testing
Sexual: persistently low libido (months, not a bad week), reduced morning erections, erectile dysfunction not explained by relationship or psychological factors.
Physical: unexplained fatigue, loss of muscle despite training, increased central body fat, gynaecomastia (breast tissue), hot flushes, loss of body hair.
Cognitive: brain fog, low motivation, depressed mood not better explained by life circumstances, poor concentration.
Symptoms that are usually NOT low testosterone
Fatigue that improves with a good night's sleep. Low libido during a stressful period at work. Mild weight gain corresponding to reduced exercise. Mood symptoms that resolved during a recent holiday. These are common, normal, reversible — and shouldn't lead to lifelong hormone treatment.
How to test correctly
Total testosterone alone is misleading. A complete assessment requires: total testosterone, free testosterone (or calculated free testosterone from SHBG), LH, FSH, prolactin, oestradiol, SHBG, full blood count, HbA1c, ferritin, vitamin D, TSH.
Timing is critical: testosterone peaks in the morning. Sample must be taken between 7am and 10am, fasting. Two separate tests on different days are required before any diagnosis of low testosterone — single-test diagnosis is one of the most common errors.
Interpreting the numbers
Total testosterone reference range varies by lab; the international consensus diagnostic threshold for hypogonadism is total testosterone below 12 nmol/L (350 ng/dL) on two morning samples, with clinical symptoms.
Free testosterone below 220 pmol/L in symptomatic men supports diagnosis. Elevated LH and FSH with low testosterone suggest primary (testicular) hypogonadism. Normal or low LH/FSH with low testosterone suggest secondary (pituitary or hypothalamic) — needs further investigation including prolactin and MRI pituitary if very low.
Treat the reversible causes first
Before starting TRT, address: sleep apnoea (very common in Gulf men, dramatically suppresses testosterone), weight loss if BMI over 30 (10 kg weight loss raises testosterone significantly), alcohol reduction, opioid medication review, anabolic steroid history, severe vitamin D deficiency correction, untreated diabetes.
Many men referred for TRT have their testosterone normalise with lifestyle and treatment of an underlying condition. TRT is for confirmed hypogonadism that persists after these steps.
Testosterone replacement therapy in Dubai: what's involved
Treatment options: testosterone gel (daily, easy to titrate), testosterone injection (every 1–2 weeks for short-acting, every 10–12 weeks for long-acting Nebido), testosterone pellets (3–6 month implant). All available in Dubai under specialist or GP prescription.
Monitoring is non-negotiable: testosterone level at 6 weeks, 3 months and then every 6–12 months. Haematocrit (red cell concentration) — TRT raises it and can require venesection. PSA at baseline and annually. Lipids, liver function. Cardiovascular risk assessment.
Side effects: polycythaemia (thick blood), worsening sleep apnoea, fertility suppression (testosterone replacement shuts down sperm production), prostate considerations, acne, fluid retention.
TRT and fertility
Critical for men under 45: standard TRT shuts down natural sperm production within 4–6 months. If you want children in future, discuss alternatives with a specialist before starting — options include clomiphene, hCG plus TRT, or fertility preservation (sperm banking) before treatment.

Full hormone assessment with two confirmatory blood draws: AED 2,000–2,800. Specialist consultation: AED 700–1,200. TRT medication monthly: AED 250–600 depending on formulation. Monitoring bloods 6-monthly: AED 600. Total first-year cost typically AED 5,000–8,000. Insurance covers diagnostic workup if symptoms documented; ongoing TRT is sometimes covered, sometimes self-pay.
