Paediatrician in Jumeirah: Fever, Rash and Ear Pain — When to Worry

A clear, parent-friendly guide to childhood illness in Jumeirah — when home care is fine, when to call a doctor, when to head straight to A&E.

Why Jumeirah families use home visits

Jumeirah villas are spread across Jumeirah 1, 2, 3, Al Wasl and Umm Suqeim. A sick toddler in a car seat for a 25-minute clinic run usually deteriorates rather than improves. A doctor at home keeps the child calm and the parent in control.

Fever — when to worry

Under 3 months with any fever above 38°C: see a doctor today. 3–6 months above 39°C, or any fever with poor feeding, lethargy, persistent vomiting, breathing difficulty, non-blanching rash, stiff neck or fever lasting more than 5 days: see a doctor today. Otherwise: paracetamol or ibuprofen by weight, fluids, monitor.

Rashes — the ones that need urgent review

Non-blanching rash (doesn't fade under glass pressure) + fever: A&E now. Sudden facial or lip swelling, breathing trouble: A&E now. Widespread rash with high fever: same-day GP. Most other rashes — viral exanthems, eczema flares, heat rash, hives — are manageable at home with antihistamine or topical cream and a single GP review.

Ear pain

Most childhood ear infections are viral and self-limit in 48–72 hours. We treat with pain relief first and reserve antibiotics for: under 2s with bilateral infection, perforated ear drum with discharge, symptoms worsening after 72 hours, or unwell with high fever. Over-prescribing antibiotics for ear infections in children is one of the most common avoidable harms in Dubai.

When a fever needs urgent review

Any infant under 3 months with a temperature 38.0°C or above. Any child of any age with drowsiness, refusing fluids, fewer wet nappies, a non-blanching rash, fast or laboured breathing, a high-pitched cry, or who simply looks unwell to a worried parent. Parental instinct is one of the most reliable clinical signs in paediatrics — we take it seriously.

What rashes mean what

Most childhood rashes are viral and self-limiting (roseola, hand-foot-and-mouth, viral exanthem). Some need treatment (impetigo, scarlet fever, eczema flares with infection). A small but critical group — non-blanching petechial or purpuric rash, especially with fever — needs immediate medical assessment to rule out meningococcal disease.

Ear pain — when and how we examine

Children pull at their ear, become irritable, sleep badly, sometimes have fever or discharge. We bring a paediatric otoscope on every home visit and can examine even a wriggling toddler with practiced technique. Most middle ear infections in children over 2 resolve without antibiotics in 2–3 days; we follow age- and severity-based guidelines rather than prescribing reflexively.

Coughs, colds and the antibiotic question

Most childhood respiratory infections are viral and self-limiting. We don't prescribe antibiotics for viral illness — it doesn't help, and it drives resistance. Where we do prescribe (confirmed strep throat, otitis media in younger children with significant symptoms, lower respiratory infections), we use targeted, narrow-spectrum agents and explain the reasoning to parents.
