Migraine in Dubai: Real Treatment, Not Just Painkillers

Modern migraine treatment has moved far beyond paracetamol. What actually works in Dubai — triptans, preventatives, CGRP injectables and trigger control.

Migraine isn't 'just a headache'

Throbbing, often one-sided pain lasting 4–72 hours, with nausea, light and sound sensitivity, sometimes preceded by aura. It's a neurological condition with a clear treatment pathway — most Dubai sufferers are dramatically under-treated, relying on paracetamol and ibuprofen that were never going to be enough.

Acute treatment that actually works

Triptans (sumatriptan, rizatriptan, eletriptan) taken at the very first warning — not when the pain is already 8/10. Combined with an anti-emetic (metoclopramide, prochlorperazine), they break most attacks in under 2 hours. We prescribe in clinic with a clear personalised plan and a sick day card.

Prevention when attacks are frequent

More than 4 days of migraine a month deserves preventative treatment. Options: propranolol, candesartan, amitriptyline, topiramate, sodium valproate (not in pregnancy). For chronic migraine, Botox injections are licensed. The newer CGRP monoclonal antibodies (erenumab, fremanezumab, galcanezumab) are now available in Dubai and are transformative for many patients — once-monthly injection, fewer side effects.

Triggers that matter in Dubai life

Dehydration in the heat, irregular meals during long workdays, screen-driven posture, caffeine peaks and crashes, alcohol (especially red wine and dark spirits), poor sleep, hormonal cycles, weekend let-down, and untreated sinus or eye strain. A headache diary for 4 weeks usually identifies the top two triggers — fixing those alone halves frequency for most people.

When migraine needs urgent assessment

First-ever severe headache, headache that peaks within seconds ('thunderclap'), headache with fever and neck stiffness, headache with new neurological symptoms (weakness, vision loss, speech change), headache after head injury, headache in pregnancy with visual changes, or progressively worsening daily headache. These are not migraine until proven otherwise — call us or go to ED.

How we manage migraine in clinic

A 30-minute consultation, headache diary review, acute plan, preventative plan if indicated, referral to neurology when needed, and CGRP injectables in clinic with insurance pre-authorisation. Follow-up at 6 and 12 weeks to measure response, not guess.

What we look for in the consult

Frequency, duration, aura presence, menstrual association, sleep pattern, hydration, caffeine intake, posture and screen time, recent medications, and family history. We use a structured headache diary for 4–6 weeks where indicated. The goal is to identify modifiable triggers and choose the right treatment ladder for the pattern.

Acute vs preventive treatment

Acute: triptans (sumatriptan, rizatriptan, eletriptan) are first-line for moderate-to-severe attacks unless contraindicated. NSAIDs and high-dose paracetamol play a supporting role. Anti-emetics help when nausea blocks oral absorption. Preventive: started when you're having more than 4 attacks per month or attacks are disabling. Options include beta-blockers, candesartan, topiramate, amitriptyline, and the newer CGRP monoclonal antibodies (erenumab, fremanezumab, galcanezumab) for refractory cases.

When to investigate further

Sudden severe headache (thunderclap), new headache after age 50, headache with fever and neck stiffness, headache with focal neurological signs, headache changing in pattern, or headache worse in the morning with vomiting — all warrant imaging and urgent specialist input.

Can a GP prescribe triptans in Dubai?

Yes — triptans are GP-prescribed for migraine after a clinical diagnosis. We confirm there are no contraindications (uncontrolled hypertension, ischaemic heart disease) and start with a single dose to test response.
