Early isn't basic. Early is where outcomes are decided. Why GPs are clinical leaders, not entry-level doctors.

The first step should never feel like the smallest

I was born and raised in England — a country where the roles of primary and secondary care were always clearly defined. You knew exactly who your GP was. You built a relationship with them. You trusted their plan. They weren't a stepping stone. They were the centre.

Fast forward to my work in the UAE and I'm seeing something very different. Here, 'early' is seen as 'basic.' Primary care is too often overlooked — even skipped entirely. And that's where the system begins to crack.

What happens when you start in the middle

Let me share what typically happens in a specialist-first approach — something we see daily. A patient experiences chest discomfort. They go straight to a cardiologist. The cardiologist orders a series of expensive tests. No critical findings. Referred to a pulmonologist. More tests. Still inconclusive. Then a gastroenterologist. Still no root cause.

Six weeks in, the patient is frustrated, confused and still unwell. Tens of thousands of dirhams spent. No clear diagnosis. No care plan. Not one of these specialists did anything wrong — but they all did what they're trained to do: look through a narrow, deep lens.

What was missing? A generalist with the full picture. Someone trained to spot patterns and simplify complexity. Someone who could connect the dots early on. A GP.

The misconception that primary care is basic

There's a dangerous assumption in the market that primary care is for minor cases — a sore throat, a flu shot, a prescription refill. But here's the truth: the earlier in your healthcare journey a professional gets involved, the more critical their thinking must be.

Genuine early diagnosis — the kind that saves lives — happens in a 20-minute consultation, not a hospital ward. GPs don't just treat. They interpret. They triage. They choose whether your next three months will be spent in recovery or in a maze of referrals. This is high-stakes decision-making, and it happens at the very beginning of your journey.

GPs as life-savers, not traffic controllers

Reframing the GP

GPs are not basic, entry-level doctors — they are clinical leaders with broad-spectrum expertise. They are not gatekeepers to specialists — they are risk managers, care planners and outcome drivers. They are not low-complexity managers — they are first-line detectors of life-threatening conditions. They are not just prescribers — they are coordinators of holistic, long-term health strategies.

This isn't about inflating the GP's role. It's about putting it where it belongs: at the top of the system, not the bottom.

The future patient will be proactive — but only if we lead the way
