Fever in Dubai: When to Treat at Home, When to Call a Doctor
Temperature thresholds, red flags, and a clear plan for what to do at home vs when to book a same-day doctor in Dubai.
What counts as a fever
Over 38°C (100.4°F) measured by ear or forehead thermometer is a fever. Most fevers in adults are caused by viral infections and settle in 3–5 days. In Dubai, where dehydration adds quickly, supportive care matters as much as the underlying treatment.
What to do at home
Paracetamol or ibuprofen for comfort (not to push the temperature to normal), small frequent fluids, light clothing, a cool room, and rest. Avoid tepid sponging and cold baths — they cause shivering and push the temperature back up.
Adults: when to see a doctor today
Fever above 39.5°C, fever lasting beyond 3 days, severe headache with neck stiffness, breathlessness, chest pain, confusion, rash that doesn't fade under pressure, recent travel to a malaria region, or any fever in pregnancy or with a chronic condition.
Children: lower threshold for review
Any fever in a baby under 3 months, fever over 39°C in any child, lethargy, refusing fluids, fewer wet nappies, breathing difficulty, a non-blanching rash, or fever lasting more than 48 hours — book a same-day paediatric review at the clinic or at home.
What temperature actually counts as a fever
38.0°C and above is a fever in adults and children. Between 37.5 and 37.9 is a low-grade temperature — often not a fever at all. Method matters: tympanic and forehead thermometers can read 0.3–0.5°C lower than core temperature. In infants under three months, anything 38.0°C or above warrants same-day medical assessment regardless of how well they look.
Red flags in adults
Temperature above 39.5°C unresponsive to paracetamol and ibuprofen, rigors lasting more than 30 minutes, severe headache with neck stiffness, breathlessness or chest pain, confusion, a non-blanching rash, or fever lasting more than 5 days without an obvious source. All of these warrant assessment that day.
Red flags in children
Drowsiness, refusing fluids, fewer wet nappies, a non-blanching rash, fast breathing, a high-pitched cry, or any infant under three months with a temperature. Children with febrile convulsions need review even when they seem to recover quickly.
What we do on a home visit
Full examination, urine dipstick if indicated, throat exam, ear exam, and pulse oximetry. We bring antibiotics, antipyretics and the kit to do same-day basic investigations. Most fevers in children turn out to be viral and self-limiting — but the value of the visit is ruling out the small number that aren't.
Dose paracetamol and ibuprofen properly
Children's paracetamol: 15 mg/kg every 4–6 hours, max 4 doses in 24 hours. Children's ibuprofen: 10 mg/kg every 6–8 hours with food, only if hydrated and over 3 months. Adults: paracetamol 1g every 6 hours, ibuprofen 400mg every 6–8 hours with food. Underdosing — common in worried parents — is the main reason fever 'doesn't respond' to medication.
When fever needs admission rather than home care
Persistent high fever unresponsive to combined paracetamol and ibuprofen at proper doses, suspected sepsis (fast heart rate, fast breathing, low BP, confusion), signs of meningitis, persistent vomiting unable to retain fluids, or any infant under 3 months with a fever. We arrange direct admission to our network hospitals with proper handover when this is the right call.

Do I need a PCR test for every fever?
No. A doctor will decide whether you need a Covid PCR, flu swab, strep swab, urine dip or bloods based on your symptoms — not every fever needs a swab.
Can I alternate paracetamol and ibuprofen?
Yes — at recommended doses, alternating every 3 hours gives smoother fever control than either alone. Always check the dose for the child's weight, not age alone.
Do I need to bring a child's temperature down?
Only if they're uncomfortable. Treating the fever doesn't change the illness duration — comfort and hydration matter more than the number on the thermometer.
