acne and irregular cycles
The combination of acne and irregular menstrual cycles often points to underlying hormonal imbalances, such as elevated androgens, which warrant clinical evaluation.
Possible causes
- 01
Polycystic Ovary Syndrome (PCOS)
commonA common endocrine disorder in women of reproductive age, characterized by irregular periods, excess androgen levels (causing acne), and polycystic ovaries.
Evidence
- NICE CKS: Polycystic ovary syndrome, 2023
- 02
Late-Onset Congenital Adrenal Hyperplasia
less commonA genetic condition affecting the adrenal glands, leading to overproduction of androgens, which mimics PCOS symptoms like acne and irregular cycles.
Evidence
- Mayo Clinic: Congenital adrenal hyperplasia, 2023
- 03
Thyroid Dysfunction
possibleAn underactive or overactive thyroid gland can disrupt the hypothalamic-pituitary-ovarian axis, leading to menstrual irregularities and skin changes.
Evidence
- NICE CKS: Hyperthyroidism / Hypothyroidism, 2022
- 04
Hyperprolactinaemia
possibleElevated prolactin levels can suppress gonadotropin-releasing hormone, causing irregular or absent periods and occasionally contributing to hormonal acne.
Evidence
- BMJ Best Practice: Evaluation of amenorrhoea, 2023
Recommended tests
Female hormone panel
→Oestradiol, progesterone, FSH, LH, prolactin, testosterone — cycle, PCOS, perimenopause.
Why: Measures LH, FSH, oestradiol, testosterone, and prolactin to identify PCOS or other endocrine causes of irregular cycles.
Guideline
- NICE CKS: PCOS - Investigation and diagnosis, 2023
Thyroid function panel
→TSH, Free T3, Free T4 — first-line test for fatigue, weight change, hair loss, mood.
Why: Excludes thyroid disorders which frequently cause menstrual cycle irregularities and skin changes.
Guideline
- NICE CKS: Thyroid disease assessment, 2022
GP consultation
→30–60 minute consultation with a British-trained GP — clinic, home or video.
Why: Provides a comprehensive clinical history, physical assessment, and personalized diagnostic plan with a British-trained GP.
Guideline
- RCGP: Quality Standards for GP Consultations, 2021
When to worry
- Sudden, severe worsening of acne accompanied by rapid hair loss or deepening of the voice (signs of virilisation).
- Complete absence of periods (amenorrhoea) for more than 3 to 6 consecutive months.
- Severe, sudden-onset pelvic pain.
- Abnormally heavy or prolonged menstrual bleeding causing dizziness or extreme fatigue.
