Wound review & dressing change

Sterile technique, wound photography for the record, assessment for infection, dehiscence or delayed healing, and escalation to your GP the same day if anything looks wrong.

Suture, clip and drain care

Suture and clip removal on schedule, drain output measurement and removal where the surgeon has authorised it, and documented handover back to your surgical team.

Observations & early-warning checks

Temperature, blood pressure, heart rate, oxygen saturation and pain score at each visit, tracked visit-to-visit so a deteriorating trend is picked up before it becomes an admission.

Medication & anticoagulant support

Administration of prescribed injections including low-molecular-weight heparin, antibiotic doses, and a review of what you are actually taking against the discharge summary.

Mobility, hygiene and recovery coaching

Safe transfers, breathing exercises, catheter or stoma support where relevant, and practical guidance for the household on what is normal in the first two weeks.

Book a doctor home visit instead of a nurse visit if you have a fever above 38.5°C, spreading redness, worsening pain, breathlessness, calf swelling or heavy bleeding — these need medical assessment and possibly a prescription change, not a dressing change. Frequently asked questions

How soon after discharge can a nurse visit?

Same day. If you send us the discharge summary before you leave hospital, we can have a nurse at your home within a few hours and align the visit schedule with your surgeon's instructions.

Can the nurse remove my stitches at home?

Yes. Suture and clip removal is routine at home, done on the date your surgeon specified, with the wound reviewed and photographed for the record before and after.

Will the same nurse come each time?

Wherever possible, yes — continuity is the point. Your visits are coordinated by one named GP, and the nurse works to that clinical plan rather than starting from scratch each visit.

Do you liaise with my surgeon?

Yes. We work to the discharge instructions and send a written summary back to the treating surgeon or hospital when there is anything they need to know.

What if my wound looks infected?

The nurse escalates immediately to the GP on duty. We can arrange a same-day doctor home visit, take a wound swab, and start antibiotics on prescription without you going back to hospital.
