Testing timing changes the answer — here's what a 4th-generation test can actually tell you at 2, 4 and 12 weeks.
The 4th-generation timeline
- · 0–2 weeks — testing rarely useful; consider PEP if within 72 hours
- · 2–4 weeks — p24 antigen may be detectable if viral load is high
- · 4 weeks — most infections detected; repeat at 12 to confirm
- · 12 weeks — a negative result is considered definitive
PEP — the 72-hour decision
Post-exposure prophylaxis is a 28-day course of antiretrovirals that reduces HIV transmission risk after a high-risk exposure. It must start within 72 hours — the sooner, the better. If you may need PEP, contact us or an emergency clinic immediately.
While you wait
Avoid sex or use condoms until the definitive 12-week result. If symptoms of acute HIV (fever, rash, sore throat, night sweats) develop 2–4 weeks after exposure, contact the clinic — the GP may bring the test forward or add HIV RNA testing.

What is the HIV window period?+
The window period is the time between HIV exposure and when a test can reliably detect infection. Modern 4th-generation antigen/antibody assays detect most infections at 4 weeks; a definitive result requires testing at 12 weeks after the last risk event. Aafiyah uses 4th-generation assays as standard.
How soon after exposure can I test?+
Testing before 2 weeks is rarely useful. From 2–4 weeks a 4th-generation test may pick up the p24 antigen if viral load is high. At 4 weeks most infections are detected; at 12 weeks a negative result is considered definitive.
Should I take PEP (post-exposure prophylaxis)?+
If the exposure was in the last 72 hours and considered high-risk, PEP (a 28-day antiretroviral course) can reduce transmission risk. This is time-critical — contact us or an emergency clinic within 72 hours. A GP will assess risk and refer appropriately.
Do I need a repeat test?+
Yes if the first test is at less than 12 weeks after exposure. A negative 4-week test is reassuring but not definitive; repeat at 12 weeks to close the window. The GP will schedule the repeat during your first consultation.
